When most people think of rehabilitation following a sports-related injury, they only think of physical therapists and athletic trainers. We follow a more comprehensive approach. Depending on your injury and your needs, we can draw upon the expertise of radiologists, orthopedic surgeons, and other healthcare professionals. We are committed not only to treating your injuries, but also to educating you about how to continue successful rehabilitation and healthy exercise after leaving our care.
Wasatch Peak therapy services partner closely – and oftentimes overlap – with inpatient therapy rehabilitation services at Davis Hospital and Medical Center. Inpatient rehabilitation services are available to meet all of our patients needs.
Davis Hospital and Medical Center offers specialized services to help our patients recover from serious illnesses and injuries such as a stroke, brain and spinal cord injuries, neurological disorders, orthopedic injuries and surgery, amputations and other debilitating conditions. Our goal is to help patients return to the highest level of mobility and independence possible.
Our hospital-based rehabilitation program provides easy access to other hospital services when necessary. Our staff includes a Medical Director, rehabilitation nurses, physical, occupational, and speech therapists, dieticians, psychologists/psychiatrists, orthotists/prosthetists, and other support staff.
Our nurses are able to accommodate patients with tube feedings, IV antibiotics, subclavian catheters, gastrostomy tubes, and other special nursing needs.
Our unit offers these inpatient therapy services:
- Audiology
- Augmentative communication
- Cognitive (brain) function rehabilitation
- Clinical and neuro-psychological evaluation and treatment
- Hearing evaluation and testing
- Lactation Services: Level 3 Newborn Intensive Care Unit
- Nutritional counseling and consultation
- Occupational therapy
- Pelvic Floor therapy
- Physical therapy
- Prosthetics and orthotics
- Recreational therapy
- Speech therapy
- Swallowing evaluation and retraining
We also provide financial case management, discharge planning and a variety of other assistance to help patients re-enter the community, such as patient/family education, social services and referrals to community resources.
Assessments are available at no charge and are used to evaluate rehabilitation potential to determine whether inpatient therapy is appropriate.

Tyler Olsen
Tyler graduated from AT Still University at the Arizona School of Health Sciences in 2007. He began his professional career working at a Skilled Nursing Facility in Salt Lake City. In 2008 he became a full-time employee at Wasatch Peak Physical therapy. He currently is the director of rehab services at Davis Hospital and Medical Center. He spends most of his time working in the hospital but does see patients in the Layton clinic. His areas of specialty include acute care, geriatrics and neuro rehabilitation working primarily with stroke survivors in the out-patient clinic. He has been a member of the American Physical Therapy Association since 2004.
Tyler and his wife along with his four children reside in Weber County. In his free time, you will find him supporting his children at soccer games, tumbling meets, choir concerts and piano recitals. He enjoys spending time on the golf course and making his yard beautiful.

Jeremy Knowles
Jeremy graduated from the University of Utah in 2012 with a Master’s Degree of Occupational Therapy. Since June 2012 he has worked at the Davis Hospital doing acute care. He also does outpatient care at the Layton Clinic as well as Home Health in Davis County. His areas of specialty include rehabilitation of patients with stroke and TBI. He also is a Lymphedema Certified Therapist and enjoys seeing people reach goals in managing and regaining function to participate in chosen activities. Jeremy has been a member of American Occupational Therapy Association, Utah Occupational Therapy Association and registered Occupational Therapist with NBCOT.
Jeremy lives in Cache Valley with his wife and 5 children. He enjoys spending time with his family outdoors, paddle boarding, kayaking, camping and taking care of the yard.

Darcy Crowford
Darcy graduated from the University of Utah with a bachelor’s in psychology in 1999. She attended Washington University in St. Louis where she received a master’s in Occupational therapy in 2002. She has experience in neurological rehabilitation, geriatrics, pediatrics, and newborn intensive care (NICU). In 2019, Darcy became a certified lactation counselor to better assist with breast feeding in preterm infants. Darcy has worked at Wasatch Peak Physical Therapy’s Layton Clinic/Davis Hospital and Medical Center since 2015. She is a member of the American Occupational Therapy Association.
Darcy resides in Davis County with her family. When she’s not keeping her energetic boys occupied at the park, doing school work, reading with them, building forts or keeping them from turning her furniture into a jungle gym, she enjoys reading, music and spending time with her husband.

Elizabeth Fanslow GiVogue
Beth graduated from Eastern Michigan University in 1988 with a BS in Occupational Therapy. Since that time Beth has worked in various areas of O.T. including inpatient rehabilitation, acute care, SNF rehabilitation, work hardening, and outpatient rehabilitation, in Michigan, North Carolina, Idaho, and Utah. Beth began working at Wasatch Peak Physical Therapy in Dec. 2014 where she focuses her skills in acute care treatment at Davis Hospital and Medical Center and in outpatient rehab in the Layton Clinic.
Beth enjoys spending time with her family biking, skiing, paddle boarding, hiking and watching college sports.

Bronda Kaschmitter
Bronda graduated from The Ohio State University’s Occupational Therapy program in 1991. Moving away from home, she took her first job at the Charlotte Institute for Rehab in North Carolina, working with adult neurological and spinal cord patients, and ultimately shifting into their pediatric rehab hospital. In 1995, Bronda joined a traveling therapy company which brought her out west to the mountains, and eventually Ogden, UT, where she met her husband. Since she has lived in Utah, Bronda has worked in several locations, including Manor Care with geriatric rehabilitation, Applegate with home health, and Morgan County school district with special needs students, grades K-12. She has been working with Wasatch Peak Physical Therapy for 20 years as a PRN therapist, and now is part-time. Bronda is certified in NDT techniques (Neuro-Developmental Treatment), and is specialized with CVA patients. Other certifications included Myofascial Release (MFR) for skeletal immobility/pain, and Early Intervention with children.
Bronda and her husband live in Ogden, where they have raised their two children- their son is now in college at Westminster and their daughter is a high school senior. Passionate about the outdoors, Bronda loves spending time with her family skiing the beautiful powder snow in the winter, hiking and mountain biking in the fall, and paddle-boarding in the summer. Whenever she gets the chance, Bronda loves to travel anywhere life has to offer- either for family vacations, backpacking, or mission trips. Being a dog lover, she thoroughly enjoys spending as much time as possible with her two standard poodles/ best friends, Nellie and Theo.

Shawn Clawson
Shawn Graduated from Utah State University in 2011 with a Masters Degree in Speech Language Pathology. He earned his Certificate of Clinical Competence (CCC) in January 2012. He has been a member of The American Speech-Language-Hearing Association (ASHA) since 2011. Shawn has worked for Wasatch Peak Physical Therapy since 2013. He works at Davis Hospital and the Layton Clinic; he will travel to all clinic sites if requested. His areas of specialties include Dysphagia, Dysarthria, Aphasia, Cognition, and working with Articulation/Phonological Disorders. He enjoys helping those who recently suffered Strokes or Traumatic Brain Injuries, along with those who have difficulty swallowing. He also enjoys working with children who need help improving their speech.
Shawn and his wife reside in Davis County where they have 3 children. In his spare time, he can be found doing anything outside with his family. He is an avid USU Aggie and LA Dodger fan. Go Aggies!! Go Dodgers!!
